Submit through Citi Mobile App ® :

• Login Citi Mobile App ®
• Select your Citi Credit Card under Card dashboard, then click “Manage”
• Click “Apply for a supplementary card”

By Mail:

Documents required:

• HKID/Passport copy of Supplementary Card Applicant
• Additional government issued identification document copy (e.g. passport or Mainland Travel Permit for Hong Kong and Macau Residents (Home Return Permit) etc.) of Supplementary Card Applicant
• Signed application form with signatures of both Principle Cardholder and Supplementary Card Applicant
